Our Service Users

Before we commence any service we always undertake an initial assessment, this usually takes the form of meeting the individual and the person’s family, talking to the person’s social worker, the school or day centre.  This helps us to understand if we can meet the needs of the person, in terms of having the knowledge and skills to support the person and the ability to manage potential risks.  We would also explore how much support is required and the practicalities surrounding this. After this initial assessment stage we will be able to advise you if we believe we can meet the needs of the person.

A full assessment will then commence, this will look in detail at the social, health, educational and functional needs of the individual and how this would be supported and indeed funded.  The information from the assessments form the basis of the individuals Person Centred Plan (PCP), this is a comprehensive document which details what is important to a person right now and for the future and what support the person needs.  On a more day-to-day level the PCP will be used to construct activity support plans and the associated structures and resources to achieve the goals in the PCP.
